This policy covers information submitted through the Unison contact form for IRB, CME, and Analytics inquiries.
Information you provide
The form collects your email address and selected department. Depending on your inquiry, it also asks for your name, phone number, role, organization, and request details. CME inquiries can include activity director and event details, or primary contact details, organization name, and consultation interests. Analytics inquiries can include your situation, assistance needs, requested deliverables, timeline, and data availability.
Required fields are marked on the form. Other fields are optional.
How your information is used
Unison uses the information to review and respond to your inquiry. Your submission is stored with a reference number, submission date, selected department, and record of your acknowledgment of the contact privacy notice.
Access to your inquiry
Access to stored inquiries is controlled by department permissions. Authorized staff can forward an inquiry to another Unison team member. Department email notifications, when enabled, include contact information and request details.
